Palliative & Home Health Nurses Association of Texas is a non profit organization, that advocates for the geriatric population with chronic diseases, aiming to improve their quality of life by providing healthcare navigation, relevant healthcare education, and support services. Each client has a PHHNAT navigator who provides comprehensive resources, eliminating unnecessary visits to the emergency room, preventing readmissions, and directing clients to alternative goods and services tailored to their specific needs.
